Hot Diggity Dog Mobile Grooming LLC Release Form
Caring for your pet is a responsibility that I take very seriously, but because I am working with living, breathing, moving animals it is necessary to have a release form signed prior to grooming.
If during the grooming process your pet becomes ill or is injured, the pet owner agrees to take the pet to the veterinarian as soon as possible at the owner’s expense, unless the injury is a direct result of negligence by Hot Diggity Dog Mobile Grooming LLC. Hot Diggity Dog Mobile Grooming LLC, it’s employees, owners, or anyone directly or indirectly related to Hot Diggity Dog Mobile Grooming LLC will not be held responsible for any sickness, injury or death caused by the pet to itself during the grooming process from pre-existing health conditions, natural disasters, or any illness a pet acquires do to never being administered or expired vaccines. Hot Diggity Dog Mobile Grooming LLC must be notified of any claims by the owner regarding compensation for veterinary care given as a result of alleged negligence on our part within 48 hours. Hot Diggity Dog Mobile Grooming LLC reserves the right to consult with the veterinarian prior to any settlement.
Hot Diggity Dog Mobile Grooming LLC will not be held responsible for clipper burn, minor nicks or cuts or irritation resulting from the grooming of matted, neglected coats or pets possessing mild to severe skin allergies including ears. Nor will Hot Diggity Dog Mobile Grooming LLC be held responsible for the stressful effects grooming may have on a pet.
Pet owners MUST inform Hot Diggity Dog Mobile Grooming LLC if your pet has bitten, or is aggressive towards people, other pets or specific grooming procedures. Muzzles may be used if necessary. Muzzling will not harm your pet and protects both the pet and the groomer. Hot Diggity Dog Mobile Grooming LLC reserves the right to refuse or stop services for such pets at any time before or during the grooming process and charge an Aggressive Dog Fee in addition to the regular grooming charge.
Hot Diggity Dog Mobile Grooming LLC reserves the right to charge additional fees for services they consider over and above the norm covered by their standard rates. Examples; severe aggression or coat condition. They also reserve the right to refuse service to customers whose pet may pose a threat to them or due to the condition of the pet (coat or age of pet) may cause unnecessary stress on the pet.
Pets with matted coats need extra attention during their grooming session. Mats left in a pets coat only grow tighter and can strangle the pet’s skin, or eventually tear it open. Heavy matting can also trap moisture and urine near the pet’s skin allowing mold, fungus or bacteria to grow causing skin irritations that existed prior to the grooming process. Torn skin from mats can also harbor maggots. Mats can be very difficult to remove and may require the pet to be shaved. When necessary, the removal of a matted coat includes risks of nicks, cuts and or abrasions. I will not be held responsible for nicks, cuts or abrasions due to shaving or cutting a matted coat.
As the owner I am responsible for the condition of my pet’s coat as presented to Hot Diggity Dog Mobile Grooming LLC and agree to release and hold harmless Hot Diggity Dog Mobile Grooming LLC in the event of any of the previously mentioned conditions.
If a check comes back as NSF (non sufficient funds) there will be a $15 fee from me plus any other bank charges.
I have read and agree to the policies of Hot Diggity Dog Mobile Grooming LLC. This contract covers all grooming from the date it is signed and all dogs authorized to be groomed by the undersigned.
